We are pleased to be your choice for your periodontal care and realize that as a new patient you may have several questions regarding our office procedures and policies. Your first appointment will be a comprehensive evaluation with a treatment plan & estimate provided for you at the end of the visit.  The fee for this first visit is $120.00 (radiographs will be additional if needed) and we will reserve an hour for your appointment and ask that you arrive 15 minutes early to complete any additional paperwork.  We file insurance as a courtesy to our patients if current information is provided. We will gladly send estimates to your insurance carrier and we will accept payment from your carrier for treatment rendered. However, we expect payment at the time of service for your portion of the estimated fees.  Please note that our financial policy requires 48 hours notice be given to cancel or reschedule an appointment. Due to the list of patients waiting for appointments, if you do not give 48 hours notice or fail to appear for your appointment, please be aware that in most circumstances you will be asked to prepay in advance of making future appointments. Should you not appear for your rescheduled appointment, the prepayment will be forfeited.

Ultimately, any balance of the bill not covered by insurance is your responsibility. Our office is a provider for Blue Cross / Blue Shield of Kansas, Cigna, and Delta Dental.

We will try to make each visit of your treatment sequence as relaxing and pleasant as possible. We offer anti-anxiety medication, laughing gas [nitrous oxide], and IV sedation, as well as head phones and video glasses for added relaxation.

Please bring your completed health questionnaire and signed copy of our financial policy, insurance card, photo ID and any radiographs your dentist may have taken, with you to your first visit. We look forward to seeing you at you first visit to the office!

Sincerely,
Drs. Edwards, Wilson, Trobough, McHale, and staff
